Hamilton Medical Center (HMC) conducts the Community Health Needs Assessment (CHNA) in accordance with the Patient Protection and Affordable Care Act (PPACA). The CHNA is designed to help the hospital determine the significant health needs of the community through quantitative data as well as input from individuals who represent the broad interests of the community.

Through the Downloads section on this page, you will find the finalized Community Health Needs Assessment, including the findings from the health data and community input process, as well as the hospital’s implementation plan to address the significant needs identified through the process. The information contained in this document is current to the best of our knowledge as of the date of Board approval, with updates to the assessment anticipated every three (3) years in accordance with the PPACA and Internal Revenue Code 501(r).
