Your Visit and what to expect
We believe we offer the best diagnostic services in Northwest Georgia.
Ask your physician to refer you to Hamilton Diagnostics Center.
You will need to complete some forms before your appointment time. Arrive at least 15 minutes early to complete the registration process. For your convenience, you can download them here and fill them out at home.

Be sure to bring them with you.
Please bring:
- All paperwork that you may have filled out or been given by your Physician
- Physician’s order
- Medication list
- Insurance cards
- Picture ID
Payment
Insurance – Hamilton Medical Center (HMC) is “in network” with many insurance carriers in our area. Please check with your insurance carrier to determine if HMC is in network with your specific plan.
We will be happy to estimate your total out of pocket expense after insurance payment. This may ( include your deductible, coinsurance and/or co-pay. You may call 706-272-6135, during normal business hours for an estimate of charges and your personal responsibility. Please be prepared to pay your out of pocket expenses on the day of service.
We will accept partial payments at the time of service and will work with you on setting up a monthly payment plan that will fit your budget.
Self Pay (No Insurance) – HMC offers a 40% prompt pay discount to patients that do not have insurance. The charges must be paid in full within 30 days of the final bill date in order to receive the 40% discount. You will be asked to pay in full or to make a partial payment on the day of service.
Concerning Your Test Results
All imaging studies are read by North Georgia Radiology
You will receive a separate bill for their services.
Results are sent to your ordering physician within 24 hours.
It is the responsibility of your physician’s office to contact you concerning the results.
