Financial Assistance – Plain Language Summary (español aquí) (see the downloads section on this page)
Hamilton Medical Center is committed to providing financial assistance to persons who have health care needs and are uninsured, underinsured, ineligible for a government program, or otherwise unable to pay for medically necessary care. Consistent with its mission to meet the health care needs of the community, Hamilton Medical Center will provide, without discrimination, care of emergency medical conditions to individuals regardless of their eligibility for financial assistance or for government assistance.
Hamilton Medical Center has established these general guidelines for providing financial assistance:
Eligibility
To qualify for financial assistance for medically necessary health care services, the patient must meet the requirements for financial assistance under the Hamilton Medical Center Financial Assistance Policy. Federal Poverty Guidelines will be used as the basis for determining whether an individual qualifies. Financial assistance ranging from a 100% discount to a 62% discount is available to qualifying individuals whose income is below 250% of the Federal Poverty Guideline. Individuals qualifying for financial assistance will not be charged more than the amounts generally billed for emergency and medically necessary care.
The necessity for medical treatment of any patient will be based on the clinical judgment of the health care provider without regard to the financial status of the patient. All patients will be treated for emergency medical conditions regardless of ability to pay or to qualify for financial assistance, in accordance with federal and state law.
Applying for Financial Assistance
Patients and families wishing to apply for financial assistance must submit an application along with supporting documentation to Hamilton Medical Center’s Patient Financial Counselor. The policy along with the application may be requested from the Patient Financial Counselor in the Patient Access Department at the hospital, located at 1200 Memorial Drive, Dalton, Georgia. The Financial Assistance Policy and Application is also available on the downloads section on this page. You may direct any questions you may have to 706.272.6018 or 706.272.6136.
