What to bring

Bring your proof of insurance and method of payment each doctor’s visit.  We accept VISA, MasterCard, Discover, American Express, cash, or check.

Financial Arrangements

You should discuss outpatient testing with your employer or private insurance carrier to determine your insurance coverage. All charges not covered by insurance are your responsibility to pay. Personal checks, cash, VISA, MasterCard, Discover, American Express are accepted forms of payment.
