Supplemental Term Life Insurance
Associates may purchase Supplemental Term Life Insurance in $10,000 increments up to a maximum of $500,000, subject to Evidence of Insurability. This maximum is separate from your hospital-provided maximum coverage. Cost for Supplemental Life Insurance is based upon your age and smoker/non-smoker status. Coverage is portable.
No Evidence of Insurability (guaranteed issue) for new employees, if enrolled during initial 30 days, limited to $150,000 employee and $50,000 spouse. Evidence of Insurability will be required for amounts above $150,000 for employee and $50,000 for spouse up to $500,000.
NOTE: Associates will not be eligible to enroll in guaranteed issue coverage for spouse life coverage for spouses age 60 and over. Evidence of Insurability will be required for any amount of coverage for a spouse age 60 and over. You may obtain an Evidence of Insurability form by contacting the Hamilton Human Resources Benefits Department.
