Confirmation of attendance to Hamilton Health Care System’s (Hamilton’s) Blueprint Gala (the “event”) constitutes agreement and acceptance of the following:

• Special Meal requests (including allergies) should be submitted at the time of your registration and no later than FIVE DAYS IN ADVANCE of the day of the event.  Requests received after this time may not be accommodated.

• All cancellation requests must be made in writing to [email protected]. Any cancellation request received fewer than ten (10) business days prior to the event (or after the event date has passed) due to an extenuating circumstance will be reviewed by the Foundation Executive Committee for approval.

10+ Business Day Notification – 100% Refund
8 Business Day Notification – 85% Refund
Seven (7) or fewer Days Notification – 0% Refund

• Substitutions (i.e., replacing one individual with another individual for the same event) will be granted. Please provide written notice to [email protected] of the change at least three (3) days prior to the event.

• There are no refunds for paid attendees who do not attend the Blueprint Gala event they were registered to attend. No credits for future events will be extended.

• Any refund requests must be received in writing to [email protected] within 30 days of the event for which the refund is requested. Any request received after this timeline will not be approved.

• I fully understand the nature of, and have an appreciation of, the risks involved with my participation in the Event and have agreed to participate at my own risk. Risks may include, but are not limited to, bodily injury (up to and including death), illness (including exposure to and infection from contagious diseases, such as COVID-19), and loss or damage to personal property. I hereby release and hold harmless Hamilton, its affiliates, parents, subsidiaries, employees, officers, directors, agents and assigns from any and all liability, actions, causes of action, suits, claims or demands for any personal injury, property damages, loss or expense incurred, by me or my heirs related to my participation in the Event.

• I agree to indemnify Hamilton, its affiliates, subsidiaries, employees, officers, directors, agents and assigns against any and all manners of actions, causes of actions, claims, demands, damages, liability and expense of any kind and nature incurred or arising from any actual or alleged wrongful act or omission, negligent or intentional, due to my participation in the Event.

• I agree to abide by all rules applicable to Hamilton Health Care System as well as any rules associated specifically with the Event.

• I hereby give my consent to Hamilton Health Care System, Inc. and its Affiliates to use my voice, quote and/or physical likeness for promotional and/or advertising purposes, including, but not limited to, televised broadcast, radio broadcast, audio/video recording, internet placement, intranet placement, news story, marketing materials or print advertising.  I have been told that this story, advertisement, and/or image will appear in the public media. I have been informed that Hamilton Health Care System, Inc. and its Affiliates may use this story, advertisement, and/or image on an ongoing basis for promotional purposes. I waive any rights, claims or interests I may have in controlling the use of my identity or likeness in the photographs, video and agree that any uses described herein may be made without compensation.  I have been informed that once information is disclosed it may no longer be protected by federal privacy regulations.
